About this service
Our Complex Clinical Care service provides specialised nursing support for participants with complex medical and disability-related needs. Our experienced registered nurses deliver comprehensive care, including PEG tube care and feeding, catheter changes and maintenance, stoma care, enteral feeding management, clinical health assessments, hospital discharge support, chronic disease management, and coordination with GPs and other healthcare professionals. We focus on delivering safe, person-centred care that promotes health, independence, and improved quality of life in the home and community.
